- The plan's face amount is less than P1 million


- The plan has lapsed for less than a year


- The plan owner & insured is one and the same person


- The plan owner is different from the insured but insured is less than 18 years old

Note: Plans for accommodation under 89-100's Phonebankers should not have an irrevocable beneficiary. Otherwise, required forms with the signature of irrevocable beneficiary should be submitted.
